Why Each Physicalist Explanation Fails

A cumulative refutation — and the moment the burden of proof shifts

The Symposium That Never Was

Imagine a symposium. The conference center is generic — a hotel ballroom, fluorescent lights, a long table at the front, name placards in front of each speaker. The audience is full. The topic, printed on the program, is direct: Explaining Near-Death Experiences: A Physicalist Symposium.

The chair welcomes everyone. He introduces the panel one by one. Jimo Borjigin from the University of Michigan, whose 2013 study on the dying rat brain remains the most-cited paper in the field.1 Rick Strassman, the New Mexico psychiatrist who ran the famous DMT trials in the 1990s.2 Kevin Nelson, the Kentucky neurologist who has argued for years that NDEs are intrusions of REM sleep into waking consciousness.3 The late Michael Persinger, represented by a former student, whose "God helmet" promised to manufacture mystical experiences with magnetic stimulation.4 A cultural anthropologist who studies how religious expectation shapes deathbed visions. And, by video link, a critical-care physician who works in anesthesia awareness.

Each speaker presents their best work. The papers are good. The slides are clean. The mechanisms proposed are not crazy. The audience nods.

Then, at the end, the chair stands at the microphone. He says: "Thank you all. I want to ask each of you the same question. I want a yes-or-no answer if possible. Here it is: Does your hypothesis explain the case of Pam Reynolds?"

A slight pause. Pam Reynolds was the woman whose head was opened, whose body was cooled to sixty degrees, whose blood was drained, whose ears were sealed with molded inserts that emitted continuous clicks measuring her brainstem response. Whose EEG was flat. Whose body had no measurable activity in the cortex during the surgery. And who, when she came back, described the bone saw she had never seen before, the conversation in the operating room she could not have heard, the small detail about her surgeon noticing her femoral arteries were too small for the bypass.5

One by one, the speakers answer.

Borjigin: "My hypothesis explains why dying brains might have a final burst of activity. It does not explain Reynolds, because the EEG was flat — there was no burst to speak of."

Strassman: "DMT might explain the visual richness of NDEs. It does not explain Reynolds, because there is no evidence the dying brain produces meaningful DMT, and it would not explain how she perceived the bone saw."

Nelson: "REM intrusion might explain dream-like elements. It does not explain Reynolds, because REM is itself an EEG-active state, and Reynolds' EEG was flat. Also, REM dreams aren't externally accurate."

Persinger's student: "Temporal-lobe stimulation can produce sensed presence. It does not explain Reynolds, because no one was stimulating Reynolds' temporal lobes — they had stopped firing — and even if they had been, local brain activity does not produce accurate distant perception."

The cultural anthropologist: "Cultural expectation shapes how NDEs are interpreted. It does not explain Reynolds' specific accurate observations, which were not the kind of thing she would have culturally expected."

The anesthesia awareness physician: "Anesthesia awareness happens when sedation is incomplete. Reynolds was not under standard anesthesia at the moment of veridical perception. Her brain was offline. There was nothing to be aware with."

Each speaker, taken alone, has done good work. None of them, taken alone, can explain Reynolds. And — this is the chapter's argument — none of them, taken together, can explain Reynolds either, because the case requires a single experiencer to do all the work, and no combination of partial mechanisms produces a single experiencer.

That symposium has not happened in real life. But the work each speaker would present has happened. The case files are real. The papers are real. And the gap between what each hypothesis explains and what the data actually require is the subject of this chapter.

What an Explanation Has to Do

Before we evaluate the remaining physicalist hypotheses, I want to slow down and say what we are looking for. What does it mean for a hypothesis to explain something? When can we say that an explanation has succeeded? And how do we judge between competing explanations of the same phenomenon?

Philosophers of science have a name for the standard most working scientists use: inference to the best explanation. The basic idea is simple. When we have several competing accounts of the same data, we prefer the account that does the best job on a handful of well-worn criteria. The first criterion is empirical adequacy: a good explanation accounts for the data we have, not just some of it. The second is unification: a good explanation uses one mechanism to handle many phenomena, rather than ten mechanisms to handle ten phenomena. The third is parsimony: when two explanations cover the same data, we prefer the one with fewer moving parts. The fourth is predictive power: a good explanation tells us what to expect when we look at new cases, and what we find should match what was predicted.6

None of these criteria is decisive on its own. A simple theory that fails to cover the data is no good. An elaborate theory that covers everything by adding new entities every time something new comes along is no good either. The criteria work together. A successful explanation does well on all four — or at least it does better on the basket than its rivals.

This matters for the chapter, because the strict physicalist account of NDEs is in trouble on several of these criteria at once. It's in trouble on empirical adequacy (the data include veridical perception, which strict physicalism does not predict and cannot explain). It's in trouble on unification (the physicalist literature has produced a half-dozen partial mechanisms, not a unified account). It's in trouble on parsimony (the cluster of partial mechanisms is sprawling, while substance dualism uses one). And it's in trouble on predictive power (it does not predict the cross-cultural consistency, the children's pattern, or the durable life-changes, all of which fall out naturally from the dualist account).

Earlier chapters have already evaluated some of the major physicalist hypotheses. Chapter 15 walked through Borjigin's dying-brain hypothesis at length and showed where it succeeds (it documents some neural activity at the boundary) and where it fails (it does not scale from rats to humans, does not explain veridical perception, and does not match the EEG data from cardiac arrest patients). Chapter 16 covered the pharmacological hypotheses — anesthesia awareness, anoxic hallucinations, endogenous DMT, ketamine-like dissociation — and reached similar conclusions. The phenomenologies don't match. The veridical content stays unexplained. The cumulative case for substance dualism is not weakened.

Three more hypotheses remain. The REM intrusion hypothesis, associated most strongly with Kevin Nelson. The temporal-lobe hypothesis, with roots in Wilder Penfield's stimulation studies of the 1950s and an unfortunate detour through Persinger's "God helmet" experiments of the 1990s and 2000s. And the cultural-conditioning hypothesis, which has been around since the earliest NDE research and which still gets reflexively invoked whenever a skeptic wants to push back on the cross-cultural data.

This chapter takes each of these three on at full strength. Then, with all the major hypotheses on the table, the chapter asks the larger question: does the cluster of physicalist hypotheses, taken together, succeed where each one alone has failed? The answer, when we reach it, is going to surprise no one who has been paying attention. But the path to it is worth walking carefully.

REM Intrusion — Kevin Nelson and the Sleep-State Hypothesis

Kevin Nelson is a working neurologist at the University of Kentucky. He has spent decades studying sleep disorders. In 2006, he and his colleagues published a paper in Neurology proposing that NDEs are intrusions of REM-sleep state into waking consciousness.7 Five years later, he developed the argument at book length in The God Impulse: Is Religion Just a Brain Function?8 The book is well-written and the argument is the strongest of its kind in print. It deserves a careful hearing.

Here is the claim, in plain language. REM sleep — rapid eye movement sleep — is a stage of sleep in which the brain is highly active and the body is partially paralyzed. Most dreaming happens in REM. In ordinary waking life, REM and waking consciousness are kept separate by brain mechanisms in the brainstem. In some people — those with narcolepsy, for example — these mechanisms can break down, and elements of REM sleep can intrude into waking life. The result is a strange mix of dream-content and waking awareness: vivid imagery, paralysis, sometimes a sense of presence in the room.9

Nelson's hypothesis is that NDEs are REM intrusions into the dying or near-dying brain. The brainstem mechanisms that normally keep REM separate from waking awareness break down under the stress of cardiac arrest or severe medical crisis. REM imagery floods in. The experiencer feels detached from the body (the partial paralysis of REM). The experiencer sees vivid imagery (REM dreaming). The experiencer feels an intense reality (REM dreams often feel intensely real). The experiencer encounters figures who feel meaningful (dream characters). And then the experiencer wakes up and reports it as an NDE.

To support the hypothesis, Nelson and his team surveyed NDErs and a control group about their lifetime experience of REM-intrusion phenomena (sleep paralysis, hypnagogic hallucinations, and so on). NDErs reported more lifetime REM-intrusion than controls. Nelson concluded that NDErs may have a constitutional susceptibility to REM intrusion that, under medical stress, produces an NDE.10

That's the case at its strongest. Now the problems.

The first problem is the most fundamental: REM intrusion does not explain veridical perception. Dreams are not externally accurate. People do not, while dreaming, perceive real-time events occurring in distant rooms. People do not, while in REM intrusion, accurately describe the surgical instruments their unconscious body is being operated on with. The phenomenon Nelson is invoking is, by its nature, an internal imaginative experience. The phenomenon NDE evidence requires us to explain is, in many cases, an external veridical perception. The two are not the same kind of phenomenon at all. The 1,114 cases of accurate distant observation in the dissertation database, and the 33 cases of accurate visual perception in the congenitally blind, are not within REM intrusion's reach.11

The second problem concerns the phenomenology itself. REM dreams have a characteristic shape: they tend to be incoherent, to shift unpredictably, to follow the loose associative logic of the sleeping mind. NDEs have a different characteristic shape: they tend to be coherent, sequential, and narratively organized. NDEs progress through identifiable stages — separation from the body, sometimes a tunnel or transition, encounter with figures, often a life review, a boundary or decision, a return. REM dreams do not progress through identifiable stages. The two phenomenologies do not match, and the mismatch is systematic.12

The third problem is timing. REM is, definitionally, an EEG-active state. The brain is firing during REM. That's most of the point — REM was named for the rapid eye movements, but it is identified physiologically by characteristic patterns of cortical activation. During cardiac arrest with documented EEG flatline, there is no REM. The brain is not running any program at all. Whatever is happening to the experiencer during the period of EEG flatline, it is not REM intrusion, because REM requires a brain that's awake enough to produce REM. The cardiac-arrest NDE cases — the strongest and most evidentially weighty cases in the literature — are not REM events.13

The fourth problem is methodological. Bruce Greyson, the University of Virginia psychiatrist whose career has been devoted to the rigorous study of NDEs, examined Nelson's data closely. Greyson and his colleagues found that the apparent correlation between NDE-experience and lifetime REM-intrusion susceptibility could be artifactual — that is, an artifact of how the survey questions were worded. When you ask NDErs whether they have ever had an "out-of-body experience" outside the NDE, many say yes — but they are often referring to dissociative episodes during the medical crisis itself, not to lifetime REM intrusion. When you separate these out, the correlation weakens substantially.14

Greyson also pointed out that even if the correlation were real, it would not mean what Nelson takes it to mean. People who have NDEs may share other constitutional features — perhaps a tendency to remember unusual experiences, or an introspective cognitive style — and these might explain both the higher REM-intrusion reports and the higher likelihood of recalling an NDE. Correlation is not mechanism.15

Common Objection

"But surely there is some overlap — some NDEs really might be partly REM-related." Yes. The dualist account does not claim that NDE phenomenology is wholly independent of brain neurochemistry. The body is the soul's instrument, and the dying body's neurochemical state may partly co-shape the phenomenology of an NDE — what colors are seen, what tunnel-imagery appears, how the transition is experienced. What the dualist account requires is not that the body has no role; it requires that the body is not the experiencer. REM-related neurochemistry might color some NDEs without being what an NDE is.

What does REM intrusion actually explain? It might explain why some NDE entry-phase reports include sleep-paralysis-like sensations. It might explain why some NDErs describe their initial separation from the body as feeling like the body is heavy or stuck. These are real phenomenological features and REM-related mechanisms might shape them. But these are exactly the features that are not the evidentially decisive ones. The evidentially decisive features — accurate distant perception, encounters with relatives unknown to be deceased, the children's pattern, the cross-cultural consistency — are precisely the features REM intrusion has nothing to say about.

So we credit REM intrusion with what it can explain, and we keep moving. The hypothesis captures something. It does not capture what we most need explained.

Temporal-Lobe Stimulation — Penfield, Persinger, and the Brain Stimulators

The temporal-lobe hypothesis has a longer history than REM intrusion and a more complicated reputation. It begins with Wilder Penfield, the great Canadian neurosurgeon who in the 1940s and 1950s pioneered the technique of stimulating exposed brain tissue during surgery to map function before resection. Penfield's surgical work was lifesaving, and his observations during these stimulations gave the world its first detailed picture of how cortical regions correspond to specific sensations and memories. He published his findings in The Mystery of the Mind (1975), a book that — in passages that have been heavily quoted on both sides of the dualism debate — concluded that the mind cannot be reduced to the brain even after a lifetime of mapping the brain's functions.16

Penfield's clinical observations are interesting in their own right. He found that stimulating certain parts of the temporal lobe could produce, in some patients, a feeling of sudden familiarity (déjà vu), or a vivid recollection of a specific past scene, or a feeling of an unseen presence in the room. None of these were full NDE-like sequences. They were fragmentary, single-feature sensations. But they showed that the temporal lobe could produce something like features that appear in NDEs.17

The temporal-lobe hypothesis, in its careful form, does not say that NDEs are temporal-lobe phenomena. It says that the brain regions that produce some NDE-like phenomenological elements are temporal-lobe regions, and that NDEs may be partly produced by temporal-lobe activity during medical crisis. This is a modest claim and largely defensible.

The trouble starts when the careful form gets stretched. Michael Persinger, a Canadian neuroscientist working out of Laurentian University in Sudbury, took the careful form and pushed it well past where it could go. Persinger built a helmet — informally known as the "God helmet" — that delivered weak transcranial magnetic stimulation to the temporal lobes of volunteers. He reported that around 80 percent of his subjects, while wearing the helmet, experienced some kind of sensed presence, mystical feeling, or out-of-body sensation.18 The press loved this. So did the New Atheists. The God helmet that proves religion is just brain function.

The trouble is that the God helmet's effects do not appear to exist. In 2005, a Swedish team led by Pehr Granqvist published a careful replication study in Neuroscience Letters. They tested Persinger's protocol on a substantial sample using a properly double-blinded design. They found no effect of the magnetic stimulation. The effects Persinger had reported were predicted not by whether the magnetic field was on or off, but by the suggestibility of the volunteers and by their expectations. In other words, the God helmet was producing whatever the volunteers expected to feel — including nothing, when they did not know whether the field was on.19

Persinger contested the replication, but the methodological consensus has settled. His original effects appear to have been the result of suggestion, not of stimulation. The "God helmet" line of evidence does not support the temporal-lobe hypothesis. Whatever the temporal-lobe hypothesis is going to lean on, it is not going to be Persinger.20

Note — How Suggestion Works in Brain-Stimulation Research

Brain-stimulation research is unusually vulnerable to suggestion effects because the volunteer is in an unfamiliar setting, wearing equipment, told that something might happen. Many ordinary sensations — the warmth of the equipment, the visual after-image of staring at a fixed point, the feeling of the room being quiet — get re-interpreted as "the stimulation effect." This is why double-blind protocols matter. The Granqvist team's contribution was to apply the same standards we use in pharmaceutical research to a magnetic-stimulation protocol. The effects vanished.

What about Penfield's clinical observations themselves? They remain valid as observations. Stimulating the temporal lobe really can produce isolated feelings of presence, déjà vu, and so on. But the temporal-lobe-elicited experiences are not actually full NDEs. They are fragmentary single-feature sensations. They lack the structured, sequential, narrative quality of an actual NDE. They lack the encounter with deceased loved ones. They lack the life review. They lack the boundary and the decision and the return. They do not include the long aftermath of changed values that NDErs report. They are bits and pieces. NDEs are wholes.21

And — the now-familiar move — the temporal-lobe hypothesis cannot explain veridical content. Local stimulation in the experiencer's brain cannot produce accurate distant observation. There is no mechanism by which firing in the right temporal lobe would tell the experiencer the color of the surgical instrument being used on their unconscious body, or the conversation in the next room, or the deceased relative's identity. The veridical content is the empirical Achilles heel of every physicalist hypothesis we have examined, and the temporal-lobe hypothesis is no exception.

Now here's where the dualist reading becomes interesting. The temporal-lobe correlations are real, and the dualist has a clean account of them. The brain is the soul's instrument during embodied life. When the instrument is stimulated, damaged, or in distress, the soul's expression through that instrument is altered. A certain kind of stimulation in the temporal lobe might produce, in the soul's experience, a feeling of presence, because the temporal lobe is the part of the instrument through which the soul ordinarily processes social-presence information. A certain kind of damage to the angular gyrus might produce, in the soul's experience, an out-of-body sensation, because the angular gyrus is the part of the instrument through which the soul ordinarily integrates body-position information.22 None of this requires the soul to be the brain. It requires only that the soul ordinarily uses the brain — which is exactly what the historic Christian theology of the body-soul relationship has said for two thousand years.23

So the temporal-lobe data are not a problem for substance dualism. They are part of what substance dualism predicts, on a careful holistic-dualist account in which the body is the soul's instrument. The data are a problem for the strict physicalist who wanted them to replace the soul. They cannot replace the soul, because they do not produce full NDEs and they do not produce veridical content.

I want to set Persinger aside and say a few words about the more careful contemporary work in this area, because it deserves better than to be tarred with Persinger's brush. The Swiss neuroscientist Olaf Blanke has spent the past two decades doing rigorous work on the neural correlates of OBE-like phenomena, and his findings are worth taking seriously. In a now-famous 2002 study, Blanke and colleagues stimulated the right angular gyrus of a patient undergoing pre-surgical evaluation for epilepsy. The patient reported a brief sensation of seeing herself "from above" — an OBE-like experience. Blanke has since developed a sophisticated account of how the integration of body-position information at the temporo-parietal junction contributes to our normal sense of being located inside the body, and how disruption of that integration can produce OBE-like sensations.22a

This is good science, and I want to be respectful of it. But two things should be said. First, Blanke himself does not claim to be reproducing full NDEs in his stimulation studies. What his stimulations produce are brief OBE-like sensations — the patient feels for a moment as if she is looking down at herself. There is no tunnel, no light, no encounter with luminous beings, no life review, no boundary, no return. The phenomenon Blanke produces is one element of a far larger pattern. Second, the stimulation-induced OBE-like sensations do not include veridical content. Blanke's patients describe feeling as if they are looking down; they do not describe accurately what is actually happening on the other side of the room or in another part of the building. The careful neurological literature on temporo-parietal junction stimulation is consistent: stimulation can produce subjective sensations of body-self mismatch, but it does not produce the full NDE phenomenon and it does not produce veridical perception.

The dualist reading accommodates Blanke's findings without strain. The angular gyrus and the temporo-parietal junction are parts of the soul's instrument through which body-position information is normally integrated. When that part of the instrument is disturbed by direct electrical stimulation, the soul's experience of being in its body is disturbed in a corresponding way. The patient feels a body-self mismatch. This is what we would expect on the dualist account, and it is fully compatible with the further claim that during a real NDE the soul is actually separated from the body and its perceptual access is no longer routed through the body's instruments at all.

Cultural Conditioning — The Hypothesis That Won't Quite Die

The cultural-conditioning hypothesis is the oldest of the physicalist alternatives to the dualist reading of NDEs. It predates Borjigin and Strassman and Nelson by decades. Its first careful form was offered in the 1970s by sociologists and anthropologists studying deathbed visions across cultures. In its strong form it says that NDEs are cultural constructs — that what people see when they are dying is shaped by what they expect to see, and that what they expect to see is shaped by their religious and cultural background.24

The hypothesis has surface appeal. Christians describe encounters with Jesus. Hindus report meeting Yama, the Lord of the Dead, or messengers from him. Buddhists report somewhat different figures and frames. Secular Westerners report a generic "being of light" that is not always identified with any specific religious figure. Surely, the cultural-conditioning theorist says, this proves that NDEs are not encounters with anything real. They are projections of the experiencer's prior religious training onto the screen of dying consciousness. What you find on the other side is what you brought with you.

The hypothesis is older than empirical NDE research, which means it has had more time to accumulate plausibility. But the actual data do not support it.

Here is the careful nuance, the move that has to be made before we can talk usefully about cultural conditioning at all. There is a difference between interpretation and phenomenology. The interpretation of an experience — what the experiencer says it means, who they identify the figures as, what religious framework they fit it into — is unavoidably culturally conditioned. Of course it is. We all think within frameworks we inherited. A Christian who encounters a luminous figure of overwhelming love and peace will identify that figure as Jesus, because Jesus is the figure of overwhelming love and peace within their frame. A Hindu who encounters the same kind of luminous figure may identify it differently. This is what we should expect.

The phenomenology, on the other hand — the actual content of what is experienced before any interpretive frame is applied — shows striking cross-cultural consistency at the core. People from radically different cultures report the same basic structure: a sense of separation from the body, a transition through some kind of darkness or tunnel-like passage, a perception of intense light, an encounter with one or more luminous beings, often a kind of life review or moral self-knowledge, a boundary or decision, a return. The figures are interpreted differently. The structure is remarkably the same.25

This is the key data point the cultural-conditioning hypothesis cannot explain. If NDE phenomenology were a cultural construct, we would expect cultural variation at every level — content, structure, interpretation, all of it. We don't see that. We see deep cross-cultural consistency at the structural level, with cultural variation showing up where we would expect cultural variation to show up: in the interpretation of the figures encountered. This is what the data actually show. It is not what the cultural-conditioning hypothesis predicts.

The earliest careful cross-cultural study was Karlis Osis and Erlendur Haraldsson's At the Hour of Death (1977), which compared deathbed visions in the United States and India.26 Despite radically different religious frameworks, the structural pattern of dying-vision experiences was strikingly consistent. Allan Kellehear extended this work in Experiences Near Death (1996), drawing on a wider range of cultures including indigenous Australian, Maori, and Native American sources.27 The same pattern: cross-cultural variation in the figures and the symbolic vocabulary, cross-cultural consistency in the underlying structure.

The dissertation's own analysis added a much larger corpus to this discussion. Across the 5,278 cases analyzed, the core phenomenological pattern — the nine-element Tabatt scoring system used in Chapter 9 — was consistent across decades, across continents, and across religious backgrounds. Christian NDErs, secular NDErs, Hindu NDErs, Buddhist NDErs, atheist NDErs all reported the same basic structure with statistically similar frequencies of core elements.28

Two further data points push hard against the cultural-conditioning hypothesis. The first is children's NDEs. Young children — three, four, five years old — report the same core pattern adults do, including elements they could not plausibly have learned: encounters with deceased relatives they had never met, descriptions of features of the medical setting they had not been told about, life reviews of their own short lives. P. M. H. Atwater's work on childhood NDEs is the major source here, and Cherie Sutherland's research on Australian childhood NDEs adds international corroboration.29 A four-year-old does not have the cultural conditioning the hypothesis requires. The pattern is there anyway.

The second is the pre-1975 NDE record. Raymond Moody published Life After Life in 1975 and effectively created "NDE" as a known concept in the public imagination. If NDEs were culturally conditioned by the Moody-and-after literature, NDE reports from before 1975 should look different. They don't. Carol Zaleski's Otherworld Journeys (1987), drawing on medieval Christian visionary literature, found the same core pattern.30 The early American writer Plato's Republic includes the Myth of Er, a soldier whose return-from-death account contains many of the standard elements.31 The pattern existed before NDE research existed. It existed in cultures that did not have a vocabulary for it. The cultural-conditioning hypothesis would predict otherwise. The data show otherwise.

Key Argument

If NDEs were culturally conditioned hallucinations, we would expect cultural variation at every level of the experience. What we actually find is striking cross-cultural and cross-temporal consistency at the structural level, with cultural variation showing up only at the interpretive level. This is exactly the pattern we would expect if there were a real underlying experience common to dying human beings, interpreted differently within different cultural frameworks. It is not what we would expect if the experience itself were the cultural product.

What about the expectation-effect literature? It is real, and we should not pretend otherwise. People do incorporate expected imagery into their reports. The expectation-effect literature is well-established in perceptual psychology generally, and it operates in NDE reporting as well. A Christian raised on stained-glass images of Jesus is more likely to describe the luminous figure they encounter as having a beard and a robe; a secular New Age experiencer is more likely to describe the same encounter as a "loving energy" or a "higher self." Some of the surface variation in NDE reports comes from this kind of expectation effect.32

But the expectation effect cannot do the work the cultural-conditioning hypothesis needs it to do. The reason is that NDErs consistently report things they did not expect. They report encounters with deceased relatives whom they did not know to be deceased — the "Peak in Darien" cases that Chapter 13 covers. They report content from medical procedures they had not anticipated and had no framework for describing. They report content that contradicts their prior religious frameworks — atheists reporting clear encounters with a luminous personal being; conservative Christians reporting universalist-sounding experiences that complicate their previous theology; Hindus reporting Christian-feeling encounters and vice versa.33 If the experience were just expectation-fulfillment, these violations of expectation should not occur. They do, and they occur often.

The cultural-conditioning hypothesis, in short, has the situation backward. It tries to explain the variation in NDE reports by saying the reports are entirely cultural products. But the actual situation is that the reports show deep underlying consistency with surface cultural variation — exactly the pattern we would expect if there were a real shared human experience that different cultures interpret differently. The variation is at the level of interpretation. The structure runs underneath.

The Matrix — Putting Every Hypothesis Against Every Phenomenon

We have now examined all the major physicalist hypotheses on NDEs. The dying-brain hypothesis (Borjigin) in Chapter 15. The pharmacological hypotheses (anesthesia awareness, anoxic hallucinations, endogenous DMT, ketamine-like dissociation) in Chapter 16. The REM-intrusion hypothesis (Nelson), the temporal-lobe hypothesis (Penfield, Persinger), and the cultural-conditioning hypothesis in this chapter so far. There are a few additional minor hypotheses sometimes added — selection bias and confirmation bias on the methodological side, and various forms of "the patient was secretly aware" arguments that try to reduce all veridical content to incomplete sedation. None of the additional minor hypotheses is more powerful than the major ones, and they fail in similar ways.

What I want to do now is set them all out together, and set out alongside them the central NDE phenomena that any successful explanation has to account for. This is the matrix the master prompt referred to. It is, in effect, a scoresheet.

The central NDE phenomena that need to be explained are these. Vivid lucid subjective experience during clinical death — that is, during periods when the brain has stopped producing the activity that strict physicalism associates with consciousness. Veridical accurate distant observation — the 1,114 cases in the database in which the experiencer reported things going on outside the body that they could not have known by ordinary means and that turned out to be correct. Visual perception in congenitally blind experiencers — the 33 cases of people blind from birth reporting accurate visual content during NDE. Encounters with deceased relatives or friends whom the experiencer did not know had died — the Peak in Darien cases. The cross-cultural and cross-temporal consistency of the core phenomenology — the same basic structure across radically different cultures and across centuries. The coherent narrative structure of the typical NDE — the orderly progression through identifiable stages, atypical of dying-brain pathology. The transformative durable aftereffects — decreased fear of death, increased compassion, reordered priorities, often vocational change, persisting for decades. The children's NDE consistency with the adult pattern. The pre-1975 consistency with the post-1975 pattern.34

Now run each hypothesis against this list. The dying-brain hypothesis explains, at most, why some neural activity might persist briefly at the boundary; it does not explain veridical content, blind NDEs, peak in Darien cases, the cross-cultural consistency, the children's pattern, or the durable aftereffects. The anoxia hypothesis explains, at most, why some experiences during low-oxygen states might feel strange; it does not explain veridical content, the coherent narrative structure (anoxia produces incoherence, not coherence), the durable aftereffects, or any of the cross-cultural data. The DMT hypothesis explains, at most, why some psychedelic-like imagery might appear; it does not explain veridical content, the cross-cultural consistency, the children's pattern (children do not have substantial endogenous DMT exposure), or the systematic phenomenological differences between DMT trips and NDEs. The ketamine hypothesis explains, at most, why some OBE-like sensations might occur; it does not explain veridical content or the moral-significance dimension. The REM-intrusion hypothesis explains, at most, why some entry-phase phenomenology might feel sleep-paralysis-like; it does not explain veridical content (REM is not externally accurate), the cardiac-arrest cases (REM requires an active brain), the coherent narrative structure, or the durable aftereffects. The temporal-lobe hypothesis explains, at most, some isolated features of presence or déjà vu; it does not explain full NDEs or veridical content. The cultural-conditioning hypothesis explains, at most, the surface variation in interpretation; it does not explain the cross-cultural structural consistency, the children's pattern, the pre-1975 cases, or the violations of expectation in actual NDE reports.

The pattern, looking down the list, is unmistakable. Every hypothesis fails on veridical content. Most fail on cross-cultural consistency. Many fail on coherent narrative structure. Many fail on the durable aftereffects. The closest any of them comes to a comprehensive account is partial coverage of the entry-phase phenomenology — a small fraction of what an NDE actually is.

Key Argument

Every physicalist hypothesis on offer fails on the central evidential category — veridical perception during clinical death. Each hypothesis captures something real about a phenomenological feature, but none scales to the full phenomenon, and the one feature they all fail to explain is the very feature that makes the empirical case for substance dualism strong. This is not a quirk of the literature. It is a structural feature of the situation: physicalist mechanisms operate inside the experiencer's brain, while veridical NDE content involves accurate perception of events outside the experiencer's brain. No mechanism in the experiencer's brain — by definition — can produce that.

The Cluster Strategy and the Sign of an Empirically Strained Paradigm

Now we step back. Each individual hypothesis fails on its own. Could the cluster of hypotheses, taken together, do better? If we accept that anoxia handles some cases, REM intrusion handles others, dying-brain activity handles still others, cultural conditioning handles surface variation, and so on, perhaps the cumulative coverage is adequate?

This is the move physicalist defenders of the literature most often make at this stage of the conversation. It is a reasonable move on its face. Many real phenomena are explained by multiple mechanisms. Disease, for instance, is explained by infectious agents in some cases, genetic factors in others, environmental exposures in others. The plurality of mechanisms is not, in itself, a sign that something has gone wrong.

But there is a critical difference between the legitimate plurality of mechanisms in a healthy research program and the desperate plurality of mechanisms in an empirically strained one. The philosopher of science Imre Lakatos described this distinction in detail. A research program is "progressive" when its core commitments lead to predictions that are subsequently confirmed by new data, and when the program incorporates new findings into a unified explanatory structure. A research program is "degenerative" when its core commitments are repeatedly contradicted by new data, and the program responds by adding ad hoc auxiliary hypotheses — additional mechanisms tacked on to handle each new anomaly without revising the core.35

The strict physicalist research program on NDEs has the marks of a degenerative program. The core commitment is that consciousness is what the brain does. The data — the veridical perception during clinical death, the blind NDEs, the children's pattern, the cross-cultural consistency — repeatedly contradict this core. The response has been not to revise the core but to add auxiliary hypotheses, one per anomaly. Anoxia for some. REM intrusion for others. Dying brain for the cardiac-arrest cases. Cultural conditioning for the cross-cultural data. DMT for the visual richness. Temporal-lobe stimulation for the sense of presence. None of these auxiliary hypotheses explains all the anomalies; each explains a subset; none of them is independently confirmed in the way the core commitment was supposed to be confirmed.

Worse, the auxiliary hypotheses are not coordinated. They are not subsumed under a single new theoretical framework. They are a list. Each one was developed by a different researcher in a different academic context to explain a different subset of the data. There is no unified physicalist account of NDEs. There is a basket of partial accounts, each of which addresses a feature, and the hope that adding them up gives a whole.

It does not. This is the deeper problem with the cluster strategy. Even if every individual mechanism captured what it claimed to capture, the cluster as a whole would still face two structural difficulties.

The first is that the cluster cannot explain why an individual NDE has the unity it has. A real NDE is one experience, undergone by one person, with internal coherence. The cluster of physicalist mechanisms — anoxia plus REM plus DMT plus temporal-lobe plus cultural conditioning, switching on and off across the experience — would produce a fragmented mess, not a coherent narrative. The experiencer's actual report is of a single sequential journey, not of jumping between mechanisms.

The second is that the cluster still does not explain veridical content. None of the mechanisms in the cluster explains how the experiencer perceives accurate distant events. Adding mechanisms together does not produce that capacity. Three things that cannot do X, when added together, still cannot do X. The veridical content remains structurally outside the reach of every physicalist mechanism on offer, and adding more mechanisms does not change that.36

The proliferation of partial physicalist explanations is not, in itself, an argument against physicalism. But the shape of the proliferation is. We have a cluster of unrelated mechanisms, each addressing a feature, none of them addressing the central evidentially weighty feature, and no unification on the horizon. This is what Lakatos called the signature of a degenerative program. When the program faces persistent anomalies and responds by multiplying ad hoc auxiliary hypotheses without unifying them under a new framework, the rational move is to consider that the core commitment may be wrong.

What about the move toward "non-reductive physicalism" — the position associated with Peter van Inwagen, Nancey Murphy, and Joel Green — which acknowledges that strict physicalism may be inadequate and seeks a richer account compatible with mental causation, intentionality, and (perhaps) consciousness during minimal brain activity? This is a serious move, and it deserves serious engagement, which the chapter on substance dualism vindicated will provide.37 But two observations are worth making here. First, non-reductive physicalism is, when carefully examined, a name for a position that gives up much of what made physicalism attractive in the first place. If the physical does not include all there is — if there are emergent mental properties that are not reducible to physical states — the "physicalist" label has been kept but the substance has shifted. Second, even non-reductive physicalism, in its current forms, has not produced a positive prediction of NDE phenomena. It is often introduced not as a new account that predicts NDE data, but as a fallback position adopted when the data exceed strict physicalism. That move is intellectually permissible. It is not an explanation in the strong sense that strict physicalism was supposed to provide.

What Substance Dualism Predicts and Why That Matters

Now turn the matrix the other way. Run the central NDE phenomena against a single alternative — substance dualism, in its careful holistic-dualist form. The position is that human beings are composed of body and soul; that the soul is not identical with the brain but normally uses the brain as its instrument; that the soul has its own perceptive faculties not exhausted by what the body's faculties supply; that at death, the soul separates from the body but continues to exist consciously.38 What does this view predict about NDEs?

It predicts subjective experience during clinical death. Of course it does. If the soul is the experiencer and the soul is not the brain, then when the brain shuts down, the experiencer continues. The phenomenon physicalism cannot explain — the lucid coherent experience during the cardiac-arrest period — is what dualism predicts.

It predicts veridical perception during clinical death. The soul, on the dualist account, has perceptive faculties not identical to the body's faculties. During the OBE phase, the soul is not seeing through the body's eyes — the body's eyes may be closed, the body's brain may be offline — but the soul has its own perceptive capacities, which the historic patristic and medieval Christian tradition called "noetic" perception.39 The 1,114 cases of accurate distant observation are exactly what dualism predicts.

It predicts visual perception in the congenitally blind during NDE. The body's visual system is non-functional in the congenitally blind, but the soul's perceptive faculties are not the body's visual system. They are the soul's. The 33 blind-NDE cases — Vicki Umipeg, Brad Barrows, and the others — are exactly what dualism predicts.

It predicts cross-cultural and cross-temporal consistency of the core phenomenology. If the underlying experience is a real common-human experience of separation and encounter, with cultural interpretive frames added on top, the structural consistency we observe is what we would expect. The variation in interpretation is also what we would expect.

It predicts coherent narrative structure. A single experiencer undergoing a real experience produces a coherent narrative, not a fragmented one. The orderly progression through identifiable stages — separation, transition, encounter, review, decision, return — is what one experiencer's testimony of one journey looks like.

It predicts the transformative durable aftereffects. If the experiencer has actually encountered a transcendent reality — has met something or someone who carries weight that exceeds anything in ordinary life — they would be expected to come back changed in deep and durable ways. They do.

It predicts that the children's pattern matches the adult pattern. The soul of a four-year-old is the same kind of thing as the soul of a forty-year-old — the body and the cognitive equipment differ, but the underlying experiencer is the same. The children's NDE pattern is what we would expect.

It predicts that the pre-1975 record matches the post-1975 record. The dying experience is a feature of human nature, not of the post-1975 cultural environment. The historical continuity is what we would expect.

One framework. Predicts every category of NDE data. This is what unification looks like in a research program. The dualist account does not need to add auxiliary hypotheses every time a new feature of NDE phenomenology shows up. The framework already covers them.40

The Standard Philosophical Objections to Substance Dualism

Substance dualism is not without its own difficulties. The chapter on substance dualism vindicated will engage them in detail.41 Let me note them briefly here so the reader is not left thinking we have ignored them.

The interaction problem. How does an immaterial soul interact with a material body? If the soul is not made of matter, how does it move muscles or receive sensory information? This was Descartes' problem and it has not gone away. The dualist response is that the interaction problem proves too much: every theory of mind faces a hard problem at the interface between conscious experience and physical matter, and the strict physicalist's "consciousness is just brain activity" is a stipulation, not an explanation. Pointing out that interaction is mysterious does not mean it does not happen. The radio analogy — that the radio is not the music but is the instrument through which the music is broadcast in this room — is one way of making the interaction less philosophically baffling.42 The careful holistic-dualist position takes the interaction as a real feature of embodied life that we observe but do not fully understand.

The neural-correlates argument. Brain damage produces mental changes; therefore mind is brain. But the careful argument here is that correlates are not identity. A radio damaged in certain ways will produce distorted music; this does not show that the music is the radio. The NDE evidence is precisely the case where the correlation breaks down — consciousness is reliably reported when the brain is doing nothing measurable. The correlation is real, and dualism predicts it, because the brain is the soul's instrument. The non-correlation cases, where consciousness exceeds brain activity, are predicted by dualism and not by the identity claim.

The parsimony argument. Why posit two substances when one might do? But parsimony is a tiebreaker between two theories that explain the data equally well. Strict physicalism does not explain the data. Substance dualism does. The parsimony argument cuts the other way once you actually look at the data. One framework that explains all the phenomena (dualism) is more parsimonious than a cluster of partial mechanisms that explain only fragments (the physicalist research program as it currently stands).

The evolutionary argument. How could a non-physical soul evolve from physical processes? But substance dualism is fully compatible with evolution of the body. The soul, on the historic Christian theological account, is created by God — not produced by evolution but given to the embodied human person.43 The body's evolution and the soul's creation are not in competition; they are operating at different levels of explanation.

None of these objections is decisive against substance dualism. Each has serious responses in the philosophical literature. The serious version of the dualism debate is between substance dualism and either non-reductive physicalism or property dualism — not between substance dualism and strict physicalism, which the NDE evidence has effectively ruled out. Chapter 23 develops these arguments at length.

What This Chapter Has and Has Not Established

Let me be careful about claims, in the spirit of the dissertation's methodological discipline. What has this chapter established?

It has established that the strict physicalist research program on NDEs has produced a cluster of partial explanations, none of which scales to the full phenomenon, all of which fail on the central evidentially weighty category of veridical perception during clinical death. It has established that the cluster strategy itself is the signature of an empirically strained research program, not the signature of a successful one. It has established that substance dualism, by contrast, predicts the central NDE phenomena from a single unified framework. And it has established that the standard philosophical objections to substance dualism, while serious, do not undo the empirical case.

What has the chapter not established? It has not established a full theology of the soul. The empirical case for substance dualism is one input into a fuller theological account; Chapter 24 and the synthesis chapters that follow develop the theological side. The chapter has not established the precise nature of the soul-body relationship — that is a longer philosophical conversation, and even within the dualist camp there are debates between Cartesian, Thomistic, and Swinburnian formulations. The chapter has not settled the eschatological questions that occupy the rest of the author's work. The NDE evidence supports substance dualism and the conscious intermediate state, but it does not on its own settle whether conditional immortality, evangelical universalism, or eternal conscious torment is the correct account of the final destiny of the unsaved. Those are separate questions, addressed elsewhere.44

What the chapter has established is more modest than a full theological program but more than a debater's score. It has established that the NDE evidence shifts the burden of proof. The reasonable default in contemporary academic philosophy of mind has been a soft physicalism — the assumption that the mental is in some way identical with or fully explained by the physical, and that any apparent counter-evidence (introspection, qualia, free will, the apparent unity of consciousness) will eventually be reduced or dissolved. The NDE evidence does not directly attack the philosophical arguments for that default, but it does empirical damage to the scientific picture that the default rests on. If consciousness is reliably reported when brain activity is absent, the picture in which consciousness is brain activity is in serious trouble. The default no longer holds without argument; it now needs an argument, and the available arguments — anoxia, dying brain, REM intrusion, temporal-lobe stimulation, cultural conditioning — do not supply what would be needed.

That is a substantial claim. It is also, the author would argue, a claim the data actually support. It is not a claim that depends on the reader's prior theological commitments. Atheist philosophers of mind, when they have engaged the NDE data carefully, have typically been moved toward something like neutral monism or panpsychism rather than back to strict physicalism — David Chalmers' work is the major example.45 Bruce Greyson, who is not a Christian, has acknowledged in his career-summary book After that the NDE data require an account of consciousness that exceeds strict physicalism.46 Sam Parnia, whose AWARE studies are the most rigorous prospective NDE research yet conducted, has developed what he calls the "lucid dying" framework precisely because the data require an account that goes beyond what classical neuroscience predicts.47 The trajectory of the empirical literature is converging on the recognition that strict physicalism cannot handle the data. What account replaces it remains contested. But the strict physicalist research program, in its classical form, is no longer where serious researchers in the field locate themselves.

I want to register one more thing about the trajectory of the field, because it matters for how this chapter's conclusion lands. Twenty-five years ago, when van Lommel's Lancet study first appeared, the standard response from the broader medical community was a kind of dismissive silence — these are interesting anecdotes, perhaps, but they cannot be what they appear to be. Ten years ago, with the AWARE I results, the response shifted toward methodological objections — interesting findings, but maybe the protocol could not sufficiently exclude residual brain activity. Five years ago, with Parnia's broader corpus, the response shifted again toward a kind of careful redescription — these phenomena are real, and we do not yet have an explanation, and we should keep studying them. The 2023 AWARE II results, and the broader "lucid dying" framework Parnia has developed since, mark a further shift. The conversation in serious cardiology, intensive-care medicine, and neurology has moved from "this cannot be happening" to "this is happening, and we do not yet know how." That shift is not yet a conversion to substance dualism. But it is a shift away from the confident dismissals of an earlier generation, and it makes the philosophical work of this chapter — and of Chapter 23 — more rather than less timely.

What This Means for Christian Physicalists in the Conditional Immortality Movement

This is the moment to say something to a particular set of readers, because the dissertation that grounds this book has them in view in a particular way.

Within the conditional immortality (CI) movement — the contemporary evangelical revival of the historic position that those who finally reject Christ are destroyed rather than tormented forever — there has been a significant turn in recent decades toward Christian physicalism. The argument runs roughly like this. The Hebrew anthropology of the Old Testament was holistic; the supposed body-soul dualism of the historic tradition is a Greek-philosophical contamination; therefore the right Christian anthropology is non-dualist; therefore the right Christian eschatology is one in which the whole person dies and is recreated at the resurrection, with no conscious intermediate state in between. Joel Green's Body, Soul, and Human Life (2008) is the major statement.48 Nancey Murphy's Bodies and Souls, or Spirited Bodies? (2006) is the parallel philosophical case.49 Glenn Peoples' work in the Rethinking Hell orbit develops the position pastorally.50

I want to engage these brothers and sisters in the most respectful way I can. Their pastoral concern is real and serious. They are not trying to undermine the gospel; they are trying to recover what they see as a more biblical anthropology and avoid what they see as Greek-philosophical accretions to the Christian tradition.

But the empirical data of this book — and of this chapter in particular — present them with a serious challenge. The strict physicalist anthropology that Christian physicalist CI rests on is the same strict physicalism that the NDE evidence has effectively falsified. If consciousness is reliably reported during periods when the brain is offline, then the consciousness in question is not produced by the brain. If the consciousness in question is not produced by the brain, then there is something — call it soul, spirit, mind, what you will — that is not exhausted by the body's neural states. That something is what the historic Christian tradition called the soul.

The Christian physicalist in CI faces three options. One: revise the strict physicalist commitment in the light of the empirical data. This is the path I recommend, and the path the dissertation argues for. CI does not require physicalism. The historic CI tradition — Edward Whately, Edward White, Le Roy Edwin Froom, Edward Fudge, John Stackhouse — has been overwhelmingly dualist or open to dualism.51 Modern Christian physicalism in CI is a mid-twentieth-century development, not the historic CI position. The CI movement can return to its dualist roots without giving up CI.

Two: develop a non-strict physicalism that is compatible with the empirical data. This is the harder path, and it requires the development of a positive account that predicts NDE phenomena rather than just accommodates them after the fact. To my knowledge no such account has yet been developed. Non-reductive physicalism in its current forms does not predict NDE data; it absorbs NDE data after the fact, which is not the same kind of explanation.

Three: reject the empirical case altogether — argue that NDEs are not what they appear to be, that the veridical content is somehow illusory, that the cumulative case has methodological flaws that disqualify it. This is the option some Christian physicalists have taken, often by appealing to global skepticism about NDE research. I do not find this option intellectually persuasive, and I think it represents the kind of motivated dismissal of empirical data that we should be careful about whatever side of a question we are on.

The book's argument, all the way through, is that option one is the right path. The recovery of substance dualism within the CI framework. The integration of the historic CI tradition with the contemporary empirical evidence. A theological anthropology that takes both the body and the soul seriously, that affirms the historic creedal "I believe in the resurrection of the body" alongside the historic creedal recognition of the soul's separability at death, and that does not require either physicalism or eternal conscious torment to make biblical sense of the human person before God.

Why This Matters for the Pastoral Theology of Dying

Let me close with what this chapter contributes to the larger argument of the book — and to its pastoral horizon. This is a synthesis chapter at the end of Part V, the part of the book devoted to evaluating the physicalist response to NDE evidence. With Part V complete, the book turns to Part VI, which is "Inside the Experience: The Stages of Dying Seen from Within."

The reason that turn matters pastorally is this. If the physicalist account of NDEs were correct — if NDEs were just artifacts of dying-brain chemistry, REM intrusion, temporal-lobe activity, and cultural conditioning — then the rest of the book would be a study of medical phenomena, useful perhaps for understanding the dying brain, but not for understanding what the dying person undergoes as a person. There would be nothing to undergo. The lights would simply go out, more or less interestingly, and the rest would be the survivor's confabulated reconstruction.

That is not what the evidence shows. The evidence shows that something real happens to the experiencer during the boundary period of death. A real person undergoes a real experience that includes accurate distant perception, encounters with deceased loved ones, a coherent narrative journey, and aftereffects so durable they reshape the rest of the experiencer's life. These are not artifacts. They are events in someone's biography — events that the experiencer remembers and integrates and lives by, often for decades afterward.

Pastoral

For pastors, hospice chaplains, and family members who sit beside the dying: the dying person is undergoing something. The "something" begins to be visible in the deathbed visions and terminal lucidity that earlier chapters covered, and in the NDE phenomenology that the next several chapters will examine in detail. What you are watching, when you watch a person die, is not merely a body shutting down. It is a person beginning a journey — one that has been described, with surprising consistency, by the millions of people who have come back from its early stages and told us what they found there. The dying person needs to be ministered to as a person, not merely tended as a body. That is what the historic Christian theology of dying always insisted on, and that is what the contemporary NDE evidence supports.

The next chapters of this book will walk into that experience. Chapter 18 takes up the OBE phase — what the soul sees from above the body. Chapter 19 follows the standard transition imagery. Chapter 20 works through what is for many NDErs the most morally significant phase, the life review. Chapter 21 takes up the encounter with the luminous figure that NDErs across cultures describe — and the question, which the chapter does not duck, of whether this figure is the Christ of the Christian tradition. Chapter 22 handles the return — why some come back, what they bring, the long aftereffects.

Without this chapter — without the careful work of showing that the physicalist explanations do not handle the data — the next several chapters would be vulnerable to a quick dismissal. Yes, you are reporting what NDErs say, but it's all just dying-brain chemistry, isn't it? The answer to that question is no, and the reasons it is no are what this chapter and the two before it have laid out. Now we can take the experience seriously as an experience.

I want to be careful not to overclaim. We do not know everything about what is happening to the dying person. The NDE evidence is evidence about the threshold — the first minutes or perhaps the first hour of the dying journey — not about the long course of postmortem existence. We do not have empirical evidence for what happens after the boundary closes for good. There the church has only the witness of Scripture and the long Christian theological tradition to guide us — and they say enough. The dying Christian goes to be with Christ. The dying soul, on the historic patristic and medieval account, is met and accompanied. The conscious intermediate state begins. The resurrection of the body remains the eschatological hope.52

But the threshold is no longer merely a matter of theological inference. It is also, now, a matter of empirical evidence. The two run in the same direction. The historic theology of dying and the contemporary NDE evidence converge on the same picture of the dying person. The body is not the whole. The soul is real. Death is a separation, not an extinction. The dying Christian is in the hands of a faithful God who does not lose what he has made.

That is what this part of the book has been arguing, and it is what the rest of the book will develop. The physicalist response to the NDE evidence has been substantial and serious, and the work of taking it seriously has been the burden of these last three chapters. The work has been worth doing. It now stands behind us. We can move into the experience itself.
