Billy was undergoing ureteroscopic surgery for kidney stone removal under general anesthesia when he experienced pulmonary aspiration during the operation. He awakened during surgery but could not open his eyes or move his body. He felt distinctly different, describing himself as feeling like a small ball of consciousness in darkness and void.

His only functioning senses were hearing and an awareness of being a small ball surrounding his intelligence. He was highly conscious and could clearly hear the doctors and nurses talking around him. He was aware he was suffering from pulmonary aspiration, which was later medically verified by the surgical team.
