On Christmas Day 1997, a patient underwent emergency valve replacement surgery for an infected heart valve. After successful valve replacement, the team could not get him off the heart-lung bypass machine. Every attempt to reduce blood flow caused his blood pressure to drop dangerously. After exhaustive efforts, the team declared him dead. The anesthesiologist turned off machines and went to eat. Dr. Rudy and assistant Dr. Cattaneo removed gowns and stood discussing the case. The body was briefly closed for autopsy. Monitoring equipment continued running, recording flatline on paper and the transesophageal echo showing no heart movement. After 20-25 minutes of no heartbeat, no blood pressure, and no heart movement on ultrasound, electrical activity suddenly appeared. The heart spontaneously restarted, generating blood pressure without any medical intervention. The patient recovered with no neurological deficit. He described floating around the operating room and accurately reported seeing Dr. Rudy and Dr. Cattaneo standing in the doorway with arms folded talking. Most remarkably, he described Post-it notes stuck on a TV monitor containing phone messages for Dr. Rudy - details impossible to know beforehand as the calls came during surgery. He reported the classic tunnel with bright light. Dr. Cattaneo confirmed in 2013: "Everything Dr. Lloyd Rudy explained is absolutely true. I was there doing this surgery. This patient had close to 20 minutes of no physiologic life whatsoever, then came back and told us what he experienced."
