On January 18, 1988, 56-year-old delivery driver Al Sullivan experienced severe chest pain at work and was rushed to the hospital. When one coronary artery became completely blocked, doctors performed urgent quadruple bypass surgery. During the operation, his heart stopped and he entered clinical death. Sullivan felt himself leave his body, moving upward through black smoke to an amphitheater-like space. From above, he watched his surgery, seeing his heart on a glass table and observing Dr. Takata making unusual arm movements that looked like flying. He saw his deceased mother and brother-in-law as beings of light. His mother guided the surgeon's hand before sending Al back to his body. Despite having his eyes taped shut and being behind surgical drapes, Sullivan accurately described specific surgical instruments, including their shape and function. He reported conversations between medical staff, including a nurse's reaction to equipment malfunction. Most remarkably, he described Dr. Takata's distinctive arm-flapping gesture, which Dr. Takata later confirmed was his unique habit. Dr. Bruce Greyson investigated the case in 1997, confirming with Dr. Takata that Sullivan's observations occurred while under general anesthesia when he could not have physically witnessed these events. The case provides exceptional veridical evidence as Sullivan perceived details impossible to observe from his position.
