During a night shift, cardiologist Dr. Richard Mansfield led resuscitation efforts for a 32-year-old man in cardiac arrest with flat EKG, no pulse, and no breathing. The team administered oxygen, 3-minute cycles of chest compressions, adrenaline, and atropine for over 30 minutes without success. Given the patient's youth, they continued briefly before Dr. Mansfield checked all monitors and connections, confirmed persistent flatline, and pronounced death. Nurses began preparing the body while Mansfield documented the case.
Fifteen minutes later, Mansfield returned to check adrenaline dosages and noticed the patient looked less blue - remarkably, he had a pulse. The team resumed resuscitation and stabilized him. A week later, the patient had fully recovered without brain damage despite approximately 15 minutes without any resuscitation after being pronounced dead. The patient then accurately described everything Mansfield had done: checking the pulse, deciding to stop, leaving the room, returning 15 minutes later, looking across at him, rechecking the pulse, and restarting resuscitation. He had observed all this while in asystole with no pulse and without resuscitation for 15 minutes. Mansfield, having carefully verified flatline, monitors, and connections before stopping, stated: "What he told me really freaked me out, and to this day I haven't told anyone because I just can't explain it." This case is exceptional for consciousness persisting not only during cardiac arrest but for 15 minutes after resuscitation ceased, with accurate observations of events during this period.
