A Dutch ICU staff member reported a remarkable case involving a photographer patient with a fresh transverse spinal cord lesion and heart rhythm disorder who experienced ventricular fibrillation requiring resuscitation. The patient was totally unconscious during the resuscitation efforts, which involved an anesthetist and three or four nurses working to restore his heart rhythm and circulation.
Immediately after the successful resuscitation, while still in the ICU, the patient provided a detailed account of the resuscitation procedures from an elevated corner viewpoint. He accurately described seeing someone holding a black balloon near his head (the bag valve mask), someone pressing on his chest for CPR, and extensive activity around his bed with many people present. Most remarkably, he correctly named each person by name and stated exactly where each team member was positioned in the room during the resuscitation. The medical team, including the anesthetist and nurses, were completely baffled by the patient's accurate observations. The case was discussed extensively afterward, not as a taboo subject but as a remarkable occurrence. The ICU staff member emphasized that someone with ventricular fibrillation has no blood circulation to the brain and drops into unconsciousness, making normal perception impossible. The patient was approximately 40 years old at the time and became wheelchair-dependent due to his spinal injury. The veridical elements are particularly strong: accurate identification of the bag valve mask as a black balloon, correct naming of all team members, precise description of their positions, and detailed observation of resuscitation procedures, all while experiencing ventricular fibrillation with no cerebral blood flow.
