An extremely obese male patient required surgery but faced multiple complications. Finding a suitable vein for catheter placement proved difficult, and mask ventilation failed to maintain adequate blood-oxygen levels. Intubation attempts through his small mouth with fleshy tongue failed repeatedly despite multiple anesthesiologists trying. Dr. Pertierra was called from another operating room while still wearing his medical headlight. He rapidly performed an emergency tracheotomy as the patient was dying. Days later, when replacing the tube, the patient told Dr. Pertierra he recognized him and had observed the entire operation from the side. After being told to go to sleep by the anesthetist, he suddenly found himself among green-clad people around his body, hearing monitors and watching the team work. He described the large tube being inserted through the tracheotomy as looking like a sword swallower act with rubber tubing. Most remarkably, he accurately observed that the anesthetist hugged Dr. Pertierra afterward, noting the relief on the anesthetist's face. Dr. Pertierra confirmed all details, including the Mayo scissors (special surgical scissors bent at the tip, unrelated to the month), his headlight, and the emotional hug from the relieved anesthetist.
