A middle-aged woman, heavy smoker and extremely obese, underwent emergency surgery for severe bronchial symptoms and respiratory tract obstruction. During the operation, she experienced secondary respiratory failure and cardiac arrest. Dr. Miguel Ángel Pertierra performed an emergency tracheotomy using a scalpel and a special trivalve tracheal dilator - an instrument resembling scissors from behind and a three-pronged bird beak from the front. After establishing artificial ventilation through a cannula, the team addressed her collapsed lung by inserting special orange needles (labeled number 14) into her chest cavity to release trapped air. While sedated and recovering, she obsessively repeated seeing the medical team and "the light." Later, she provided Dr. Pertierra with remarkably detailed observations. She described watching from behind him as he cut her neck with a scalpel, requested something by number, and received "strange scissors that opened downward in three parts." She accurately described the white plastic tube insertion and the connection to ventilation equipment. She observed the team's response to monitor alarms and their insertion of "huge needles that were orange where they were widest." She experienced this from about one meter away while simultaneously feeling the needle insertion as the most painful part. Dr. Pertierra confirmed her observations had no normal explanation, particularly noting she observed extremely specialized otolaryngological instruments unfamiliar to laypeople, and the needle color is never verbally mentioned when requested.
