As a young child, the subject was playing with a doll in the basement when their family dog (Collie/German Shepherd mix) began growling at them from the stairs. Despite calling for their mother, they received no response. Attempting to climb the stairs, the dog attacked from behind, biting them in the back of the head. Their mother found them, stopped bleeding with water, wrapped them in a blanket, and rushed to the hospital. The subject's grandfather (a doctor) and grandmother (a nurse) operated on them. Under ether anesthesia administered through a black rubber mask, they counted backward from 100 to 97. They experienced an out-of-body state floating in a void of complete darkness. Though everything was black, they could see clearly all around as if there was light. The space was so soundless it hurt their ears. They sensed invisible boundaries they couldn't cross and called out "Isn't anyone there?" multiple times with increasing anxiety. No one answered, and they feared being alone forever. They suddenly woke in their parents' bed at night, immediately asking "Mommy what is it like to die?" Their mother said nothing. The grandfather later revealed the bite went through the skull, missing the brain by a quarter inch. The dog was euthanized immediately. The subject was swollen for over a month and remained terrified of animals for years. They now think about death every day.
