Anthony was shot in the head with a .30 caliber rifle round on April 2, 2004. The bullet hit a piece of metal next to him before striking his face with multiple fragments, knocking him back from the table where he was sitting and rendering him unconscious. At the hospital, approximately 50 metal fragments were found in his face and skull, which worked out of his body over the years.
During the unconscious period, Anthony experienced himself floating above his body, watching the scene from above. He saw multiple people kneeling around him trying to stop the bleeding. Everything appeared in black and white rather than color. From this elevated perspective, he had no pain and was confused about what had happened, thinking "Am I dead? Did I just get killed? What happened?" He was particularly concerned about his wife and child. Time seemed to stop during this out-of-body observation. The primary veridical observation was seeing himself from above while people worked to stop his bleeding. He could see the entire scene from an elevated perspective that would have been impossible from his unconscious position on the ground. He distinctly remembers seeing multiple people kneeling around his body and their efforts to control the hemorrhaging. Soon after the out-of-body experience, he found himself back in his body looking up at the sky while people placed towels over his face to stop the bleeding. He recovered after a few weeks and noted that he remembers this experience more accurately than other life events from that time period, stating it feels "like it was yesterday." Despite the traumatic nature of the injury, the experience included both distressing elements (concern for family) and a sense of calm relief during the out-of-body state.
