A woman in her 40s came to the emergency room in cardiac arrest and was immediately taken to the catheterization lab, where a nearly complete blockage of her left main and LAD arteries was discovered. After the catheterization, she was transferred to the ICU where the reporting nurse cared for her. Just as the nurse was about to get the patient's family, the patient grabbed her scrubs saying "Something isn't right" and went into ventricular fibrillation. The nurse began CPR and called the code team, who worked extensively to revive her, with the nurse alternating between compressions and documentation.
During the code, the medical team made jokes and puns to relieve stress, including the pharmacist who stood in the doorway asking what "a drug dealer" could do for them. The nurse looked down at the patient saying "Come back! You're way too young" and "I'm not going to let you die on my watch!" The patient eventually regained a pulse and spent several days in critical care. When extubated, she reported an out-of-body experience where she floated at the ceiling with a childhood friend who had passed away. She accurately described where every person was standing in the room and repeated some of the puns that were made. The patient said her deceased friend explained that making jokes was how medical professionals deal with stress. She saw a soothing light and felt herself floating toward it until she heard the nurse yelling not to die on her watch, which she believes brought her back. She later called the pharmacist a "drug dealer" based on what she heard during the code. This experience completely changed how the nurse cares for comatose and arrest patients, knowing they may be aware of their surroundings.
