Derek experienced cardiac arrest in March 2009 following a heart attack months earlier. While sitting in his garage, he stood up, felt lightheaded, sat down, and lost consciousness. During the event, he found himself in a place of extraordinary light, different from normal light, where he encountered his deceased mother (d. 1980), grandmother, and grandfather (d. 2007/2008). Derek was lying on a floor-like surface when his relatives expressed shock at seeing him there. His grandfather approached and asked "What about the kids?" prompting Derek to think about his children. He realized he could exist consciously without breathing and felt he was given a choice to return. When he chose to come back for his children's sake, he immediately regained consciousness on the garage floor with a split and bleeding wound over his right eye. Derek, who lost his mother at age 11, interpreted his grandfather's reminder about his children as relating to his own experience growing up without a parent. The experience left him certain of an afterlife and eliminated his fear of death.
