Phil was swimming at Josephine Falls with his wife and daughter, repeatedly riding down a natural rock waterslide. During his last ride, he attempted to ski down on his feet but slipped and hit his head before falling into the deep pool below. He remained unconscious underwater for an estimated 5+ minutes. During this time, he experienced being underwater in an endless, peaceful pool with crystal-clear water and beautiful light patterns. He then had dual awareness - both underwater and viewing the scene from above. He encountered his deceased sister Diane who had died 13 years earlier from cancer. She appeared radiant and fairy-like, beckoning him to follow her through the shimmering water. They swam together until she headed upward, at which point he was pulled back to consciousness. Two men had dived in to rescue him after he finally surfaced. He was diagnosed with severe concussion and head lacerations but recovered without serious physical effects.
