Keith experienced four major heart attacks on New Year's Eve 1991, flat-lining twice with one episode lasting 4.5 minutes. He immediately traveled out of body to the ceiling, observing medical staff working on him below. Dark formless creatures emerged from the hospital floor, trying unsuccessfully to grab him as he rose, filling him with fear. He then encountered an otherworldly light described as the most "glorious of all Florida sunsets" with perfect peaches, corals, roses and bright yellows. Rising to these sunset colors, Keith underwent a life review where he was instructed to "judge himself," which filled him with shame as everything was revealed. Meeting directly with God, Keith asked "Where am I?" and was told "You are dead." Keith argued, saying "But, I have found love for the first time in my life and I do not want to be here." God responded: "You will go back through terrible pain. Now go back and save your people." Upon returning to his body, Keith punched the attending doctor, initially perceiving him as a car driver who had hit him. The experience included a secondary account where a security guard named Pat approached Keith's wife Kim in Atlantic City, stating she had been shown Kim's face during her own NDE and told to "find Kim."
