Fred S underwent oral surgery for a maxillary sinus uplift with twilight anesthesia. Having an abnormally low heart rate (42-48 BPM), he had a violent reaction to the anesthesia. He felt paralysis creeping up his body, lost the ability to breathe, and tried to scream but could not. He described feeling like being nailed inside a coffin. After relaxing, he left his body and floated above himself, observing his stretched-out form. A dental technician saw his distress and yanked out the IV tubes.
